Adult Day Program

Balance provides a community-based Day Program, Monday through Friday, for adults with intellectual and developmental disabilities (IDD). 

Located in Grafton, Wisconsin, Balance’s Adult Day Program focuses on addressing the disparities that people with IDD face daily. Our goal is to improve each person’s quality of life by involving them in activities that take them out into the community at least once, and often two or more times per day.

Daily activities serve to enrich health and wellness, bolster creativity and socialization, and give participants opportunities to connect to their community through various outings and volunteer opportunities.

Program participants are supported in 1:1 care, 2:1 care (2 participants with 1 staff), or a small group of 4-5 individuals with one staff member.

Participants spend approximately 75% of their time out in the surrounding communities.

A typical day consists of:

  • Arrival at Balance, social time

  • Morning community outing

  • Lunch at Balance’s office in Grafton

  • Afternoon community outing

  • Return to Balance, socialize with friends, and get ready to return home

Individuals may begin Adult Day programming when they turn 18 years old.

Balance also offers a 10-week Summer Day Program, running from mid-June through mid-August, for individuals who need assistance transitioning out of high school and into an Adult Day Program.

Adult Day Program Admission Process

Admission to Balance's Day Program is initiated by the family contacting the Director of Day Programs and working with and through their selected Family Care Organization or IRIS.

Balance will then initiate "Pre-Admission" and will give the family paperwork to complete that will provide the Day Program with important information about the individual.

The Balance Director of Day Programs will reach out to the family and the appropriate Care Manager to discuss the admission application. If an opening is available, and if the individual appears to be a good fit for the Day Program, an in-person visit and tour of the Day Program will be scheduled.

If it is determined that the individual is a good fit, full admission will be coordinated with the family and Management Care Organization or IRIS.

Based on the expertise and current training of Balance staff, the following do not fit our capabilities:

• Significant/Severe behavioral issues

• Individuals who need nursing or medical care

• Individuals who need injections that require staff administration

• Medical or physical limitations that preclude the individual from regular community integration
